Tri-County Nursing Home has become a reality because of the pressing need for a first-class nursing facility in the Lower Suwannee River Valley embracing the Florida counties of Gilchrist, Dixie, and Levy. Our Skilled Nursing units provide round-the-clock attention from compassionate, licensed nurses. Residents receive quality nursing care and other health-related services by trained and sensitive professionals. Our facility offers intensive rehabilitative therapy in our fully-equipped therapy departments. Our licensed therapists’ primary goal is to overcome or minimize each disability and to restore all patients to their fullest potential. Residents receive the finest nursing care available while they recuperate to full strength in our comfortable rooms. We also provide Outpatient Physical and Occupational Therapy.
They also enjoy our sunny courtyards, community lounge and dining room. Friendly neighbors and a relaxed attitude make what could be a difficult period an enjoyable experience. At Tri-County we are determined to make your stay as pleasant as possible; that’s why we include not only the necessities but all those nice extras - such as the arts and crafts room, a delightful gazebo, beautiful Florida landscaping and a helping hand attached to a caring human being where and whenever it’s needed. You can count on Tri-County! Our Intermediate Care unit is designed especially for those in need of less intensive nursing care. Licensed nurses will supervise and assist with medication administration, ambulation and other daily living activities to the degree appropriate.

Tri-County Nursing Home Awarded Governor’s Gold Seal for Excellence in Long-Term Care.
Tri-County Nursing Home has been recognized for excellence in care management and resident quality of life by the Governor’s Panel on Excellence in Long-Term Care. This marks the first time we have received this prestigious honor.
Of nearly 700 nursing homes in the state, only 16 currently hold the Governor’s Gold Seal Award. Tri-County Nursing Home is the only facility within an 85-mile radius to earn this distinction.
The Gold Seal Award recognizes nursing homes that demonstrate consistent excellence in long-term care, promote workforce stability, and enhance the physical, social, and emotional well-being of their residents.

The Gold Seal Award program was established in 2002 and is implemented by the Governor’s Panel on Excellence in Long-Term Care, composed of representatives from the Governor’s Office, Agency for Health Care Administration (AHCA), Department of Health, Department of Elder Affairs, Florida Health Care Association (FHCA), Florida Life Care Residents Association (FLCRA), LeadingAge Florida, and the State Long-Term Care Ombudsman.
In addition to the Gold Seal, Tri-County Nursing Home holds Joint Commission Accreditation, a 5-Star rating from the Centers for Medicare & Medicaid Services (CMS), a 5-Star rating from AHCA, and is recognized by U.S. News & World Report as a Top Performing Nursing Home in both short- and long-term care.
